BREA Forms by Application Type

BREA's forms are all in PDF format unless otherwise specified. Many forms have fields into which you can enter your data using the free Adobe Acrobat Reader. This form can then be printed, but the data is not saved when the document is closed unless you have the full Adobe Acrobat or Acrobat Business Tools software.

Initial Trainee (AT)

• Initial Application (Form 3001)
• Basic Education Attachment (Form 3002)
  Include copies of course completion certificates with this application
  Include a copy of the course completion certificate for California Laws & Regulations
• Request for Live Scan Service (BCIA 8016)
  

  If you reside in California, please download this form, fill out the section labeled "Applicant Information", and take this form with you to get fingerprinted at a Live Scan facility. Return a copy of the completed form with your Initial Application package.

  If you reside outside of California, you will need to be fingerprinted on the federal standard fingerprint card FD-258 and submit two (2) of these cards with your Initial Application package.

Initial (All Others - AL, AR & AG)

• Initial Application (Form 3001)
• Basic Education Attachment (Form 3002)
  Include copies of course completion certificates with this application
  Include a copy of the course completion certificate for California Laws & Regulations
• Log of Appraisal Experience (Form 3004)
• Request for Live Scan Service (BCIA 8016)

• Continuing Education Attachment (Form 3017) (Optional)
  Include copies of course completion certificates with this application

Reciprocal

• Reciprocal Application (Form 3025)
• Consent to Service of Process (Form 3006)
• Include a copy of the course completion certificate for California Laws & Regulations

• Request for Temporary Practice Permit (Form 3009)
• Consent to Service of Process (Form 3006)
• Include a copy of the letter of engagement that clearly identifies the property or properties to be appraised.
